Policies and Forms
Students wishing to enroll in the course need to complete the
course enrollment form and return. Students may e-mail
this form to
firstname.lastname@example.org or may call to obtain mailing
address or fax number. Full payment is expected at
enrollment unless an alternate arrangement is made. I
accept payment in (order of preference) cash, money order,
personal or business check, firearms and ammunition, game
Classroom- No ammunition is
permitted in the classroom for any reason.
If you carry a loaded firearm to the classroom please be
sure to unload your firearm before entering. Please
arrive about 15 minutes early to check in and complete any
necessary paperwork. Course will begin promptly at the
start time. Access to classroom requires the ability
to climb and descent a set of stairs, if you cannot for any
reason - please contact me for alternate arrangements.
bring an unloaded personal firearm, if available to the
classroom. Be sure to bring a copy of government
issued photo ID to the course. This is required for
both the classroom portion and range portion of the class.
Students should also bring a pen and notepad.
72 hours notice for a full refund. If a cancellation
is received after 72 hours but before 18 hours refund will
be given in the form of a credit towards another course.
If a no waiting list exists a no-call-no-show the student
will be given a 50% refund of the enrollment fee. If a
waiting list exists a no-call-no-show will result in
forfeiture of the entire course enrollment fee.
minimum age for the Utah Multi-State Concealed Firearms
Course is 20Ĺ. For all other courses there is no
minimum age, however sound judgment will be exercised.
Students who are under 18 may enroll but a parent or legal
guardian must be present for the entire classroom and range
portions of the course. All courses are for those legally
able to own/possess a firearm. If you are under any
legal disability which prevents you from owning or
possessing a firearm you may not enroll in this course.
